Subcontractor information and subrecipient required forms

If you represent a subcontractor organization to UDRI, please use the instructions below to obtain, complete, and return the necessary forms.

As specified in separate correspondence from the UDRI Subcontracts Office, please select the applicable forms from the list below to complete and return to subcontracts@udri.shushijia.net.

All Awards

Completion of the following audit certification forms are requested by the University on an annual basis.

• Non-Profit Audit Certification (complete only if non-profit)
• For-Profit Audit Certification (complete only if for-profit)

Federal Awards

Completion of the following FAR / DFARS forms are requested to be completed for federal awards prior to fully executing a subcontract with the University. 

• DFARS 252.204-7020 – NIST SP 800-171 - DoD Assessment Requirements (complete if SPRS score is below 80)
• DFARS 252.227-7013 & 252.227-7014 – Assertion of Restrictions for Technical Data and Computer Software (MAR 2023) [corresponding FAR Clause: 52.227-15]
• FAR 52.203-11 – Certification and Disclosure Regarding Payments to Influence Certain Federal Transactions - Lobbying (SEPT 2024) (complete if subcontract award is ≥ $150k)
• FAR 52.204-24 - Representation Regarding Certain Telecommunications and Video Surveillance Services or Equipment (NOV 2021)
• FAR 52.204-10 – UDRI First-Tier Subcontractor Transparency Act Reporting Form (MAR 2023) (complete if subcontract award is ≥ $30k)
• FAR 52.209-5 – Certification Regarding Responsibility Matters (AUG 2020) (complete if subcontract award is ≥ $250k)
• FAR 52.209-11 – Representation by Corporations Regarding Unpaid Tax Liability (FEB 2016)
• FAR 15.403-4 – Certification of Current Cost or Pricing Data (complete if subcontract award ≥ $2M)
